We are dedicated to providing compassionate, high-quality end-of-life care throughout San Antonio and surrounding communities. Responsibilities
- Provide skilled nursing care to hospice patients in homes, facilities, assisted living communities, and other care settings in accordance with the plan of care
- Participate in an on-call rotation
- Perform routine visits, admissions, recertifications, PRN visits, and death visits as needed
- Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team
- Administer medications and treatments as ordered while ensuring patient comfort and safety
- Monitor changes in condition and communicate updates to physicians and team members promptly
- Educate patients and families regarding hospice services, symptom management, coping strategies, and end-of-life care options
- Support the emotional and spiritual needs of patients and families during this critical time
- Complete all clinical documentation accurately and within 24 hours of each visit
- Participate in biweekly IDG (Interdisciplinary Group) meetings
- Maintain compliance with Medicare, state regulations, and company policies
- Provide after-hours support during assigned on-call rotation
Qualifications
- Current and active Texas Registered Nurse license in good standing
- Minimum 2 years of nursing experience required
- Minimum 1 year of Hospice Nursing experience required
- Strong clinical assessment, time management, and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently while collaborating effectively with a team
- Compassionate, patient-centered approach to care
- Current BLS/CPR certification required
- Reliable transportation, valid driver’s license, and current auto insurance required
- Experience with EMR systems preferred (Axxess experience a plus)
